In accordance with the GDPR and Data Protection Act 2018 I have a legal responsibility to all my clients to inform them what; how and why I am collating or retaining data about them as individuals. What - Name; Contact Details (Including an emergency contact), Medical data (Including surgery and GP details) This is an opportunity for you to disclose any existing conditions/diagnosis which may/may not be of importance for me to be aware of, during our sessions. Content notes that I take following our sessions.
How - All data is kept electronically and encrypted with a subsequent password. Please note that all clients and their data are kept in a folder which is given a unique code, preventing anyone other than myself from being able to identify them.
Why - For your and my safety and to enable me to carry out my role as your therapist in a professional manner and in accordance with my associations standards and expectations. All data is deleted when no longer required, in terms of relevancy or if we are no longer in a therapeutic alliance.
